Air pollution is filed under lungs. The image that comes with it is a cough, an inhaler, a chest X-ray.
The World Health Organization publishes what people actually die of, and the list is in a different order.

The routeWhy the wrong organ is famous
The lungs are where the pollution enters. They are not where most of the damage is counted.
WHO states the mechanism plainly: the mortality “is due to exposure to fine particulate matter, which causes cardiovascular and respiratory disease, and cancers”. Fine particles are small enough to leave the lung and enter the bloodstream, and once there, the organs they reach are counted in the first line of the list, not the last.
Why the order matters. A risk understood as a lung problem gets compared with smoking, discussed with respiratory specialists, and worried about by people who already cough. A risk that is mostly cardiovascular belongs in a different conversation entirely, the same one as blood pressure and stroke prevention. The WHO's own sentence closes on that point: its latest burden estimates “reflect the significant role air pollution plays in cardiovascular illness and death”.

Two numbers, and the difference between them is where the pollution is.
The difference between the two is household air pollution, cooking, heating and lighting indoors. WHO counts it separately at 2.9 million deaths in 2021. It is the same subject at a different address, and it kills on a comparable scale.
Do not add them up. 4.2 million (outdoor, 2019) plus 2.9 million (household, 2021) is 7.1, but WHO's combined figure is 6.7 million. The combined total is deliberately lower than the sum, because the same people are exposed to both and would otherwise be counted twice.
Two figures that describe overlapping populations cannot be added. We say so because the arithmetic is tempting and wrong.

Two percentages describe the same inequality from opposite ends.
- 99% of the world's population lived, in 2019, in places where WHO air quality guideline levels were not met. Breathing air that meets the guidance is the rare case, not the normal one.
- 89% of the premature deaths occurred in low- and middle-income countries, with the greatest number in the WHO South-East Asia and Western Pacific Regions.
Almost everyone is exposed; the dying is concentrated. WHO writes that people in low- and middle-income countries “disproportionately experience the burden”the exposure is near-universal, the consequences are not.

Most environmental subjects end with a list of things a reader can change. This one ends with a sentence from WHO explaining why that list is short.
“Most sources of outdoor air pollution are well beyond the control of individuals and this demands concerted action by local, national and regional level policy-makers working in sectors like energy, transport, waste management, urban planning and agriculture.”
The same paragraph calls air pollution the second highest risk factor for noncommunicable diseases.
Five sectors are named. None of them is a household.
That is not a reason to do nothing, WHO's own policy list is long and specific, from clean household energy to rapid public transport to methane capture at landfills. It is a reason to aim the effort where the sources are, and to stop measuring a person's seriousness about air quality by what they do in their own kitchen.
